At this time, I am only in-network with DMBA, Blue Cross of Idaho, and Mountain Health Co-Op. I am in the process of getting credentialed with additional insurance companies in Idaho, but this process takes time. As I become in-network with different insurance companies, that will be reflected here.

As a courtesy, I would be happy to provide you with a Superbill/Receipts that you can submit to your insurance company for possible reimbursement or to go towards your deductible.
